Refills: Please call your pharmacy and ask them to generate an e-mail or fax request and be sure that they have our new fax number (708) 589-9059. If its an urgent need for a refills, please call the main office number.
Billing Questions: Please call the office number above and ask to be transferred to the billing office. Online bill payment is available through the patient portal.

After hours the general office number will forward to our Answering System. Please follow the instructions with your phone keypad, and it will allow you to create a message that will forward to your provider.
E-mail: Please e-mail us through the secure patient portal. As e-mails are only checked on a periodic basis throughout the day, we require that e-mails are restricted to non-emergent questions only.
